PARENTS OF sixth, seventh and eighth students 11 years and older: Remember to turn in your medical records for the newly required Tdap & Meningococcal immunizations.
ALL STUDENTS: All students are now required to have a Varicella (chicken pox) immunization if they have not had the disease of chicken pox. Please notify me if your child has had chickenpox.

All immunizations MUST be current to begin attending school and kept compliant as additional immunizations may be required.
There has been a change in the SUSD guidelines regarding the over-the counter medication that is given out in our School Health Centers. You now need to complete a MEDICATION ADMINISTRATION RECORD, HMES 503, one form for each medication. Also, you must provide the specific, age-appropriate medication (as listed on the medication) and it must be in an original, sealed container before any over-the-counter medication will be given to your child. More information is listed under the MEDICATION POLICY, Important Information column below. Let me know if I may be of assistance for you bringing medication for your student.
