arrow.gifSUSD Middle School Athletic Code of Conduct signed by both the athlete and parent/guardian.

The Physical Evaluation and SUSD Middle School Athletic Code of Conduct forms will be valid for the 2016-17 school year.  However, the two parent consent cards must be submitted for each sport.  See all forms and information below.  You may print, complete, and submit these forms to Mr. Sampson.

arrow.gifThe Tier 1 athletic fee is $60 per sport and must be paid prior to the start of the All Included Tier 1 sport season. 
arrow.gifThe $30 athletic fee for the Tier 2 Competitive level must be paid prior to the first contest and before any game equipment is issued.
arrow.gifThe fee for Track and Field is $60 for the entire season.
arrow.gifArizona tax credit laws do not allow for refunds.

  These forms will also be available from the Athletic Director and in the front office prior to that specific sport season.  Please print, complete, attach payment, and return to Mr. Sampson.  There are no refunds of the AZ Tax Credit Participation Fees.
