A physical evaluation must have been completed after March 1, 2016; the form signed and dated by a licensed physician.
TWO (2) parent consent cards must be completed and signed by the student and parent/guardian.
Insurance information is required.
SUSD Middle School Athletic Code of Conduct signed by both the athlete and parent/guardian.
The Tier 1 athletic fee is $60 per sport and must be paid prior to the start of the All Included Tier 1 sport season.
The $30 athletic fee for the Tier 2 Competitive level must be paid prior to the first contest and before any game equipment is issued.
The fee for Track and Field is $60 for the entire season.
Arizona tax credit laws do not allow for refunds.
These forms will also be available from the Athletic Director and in the front office prior to that specific sport season. Please print, complete, attach payment, and return to Mr. Sampson. There are no refunds of the AZ Tax Credit Participation Fees.
